 The AMCIS Network brings a practice the ability to provide better continuity of care.
One of the ways we accomplish this is through clinical documentation in the Patient Care Manager.
AMCIS Documentation begins during the installation setup. Our implementation staff train you to quickly and easily set-up basic templates that tell AMCIS what kind of notes you like to write and what note sections you want to include. You can modify existing templates or start from scratch. After setting up your note templates, you will set-up your diagnosis based templates. Again, this is an easy process and you can use existing clinical content.
When you are ready to write your note:
- Click on the patient's name on your home page schedule.
- Click new note and select the note template you would like to use.
- Click as many diagnosis templates as needed for this patient encounter.
At this stage, your note will be nearly completion in prose:
- The History of Present Illness, Physical Exam, Assessment and Plan will contain specific information for the selected diagnosis.
- The Past Medical History, Social History & Medical History will populate your note.
- The Medications and Allergies will populate your note.
- Click on the specific ICD9 code(s) for your encounter and submit.
- The entire note is now editable. Notes can be edited in multiple ways:
- voice recognition,
- pick lists,
- free-edit typing,
- transcription, and
- free-form hand writing
A great portion of the note can be pre-entered by staff or by the patient using the Healthport. Patients enter their past medical and surgical history, family and social history, allergies, medications and review of systems into their eChart. All of that clinical information goes directly into the physicians note in prose in the AMCIS Patient Care Manager, without a computer generated appearance.
If a patient is returning to the office, their prior note can be renewed which saves the provider a great deal of time documenting the new encounter.
To renew a note:
- Check the prior note and click submit.
- Select the new note template -- Maybe you are renewing a New Patient note to an Office Visit.
- Click on any templates for new diagnoses.
- Check the parts of the prior note that you want to keep.
- Edit the note as desired.
Notes can be automatically faxed to the patient's referring providers upon signature. Often the notes arrive at the referring provider’s office before the patient has left the parking lot.
M.D. Web Solutions offers a unique note migration service, wherein thousands of previously transcribed notes can be transferred into AMCIS in renewable format. Providers who use this service can renew notes that they previously had transcribed for returning patients during the first day of using the AMCIS Patient Care Manager.
Evaluation and Management (E&M) Level determination and CPT coding can be chosen by the provider upon signing the note or by staff after the note is signed. This information will flow to the billing system.
